[Bug] Codex 突然開始回傳「empty/malformed response」錯誤
回報者原本使用 openai/codex 模型一整天都正常，但之後每則訊息都收到「Empty/malformed response，切換到 fallback」的錯誤，即使切換其他 OpenAI 模型也一樣，只有 Anthropic 等其他 provider 正常。回報者嘗試重新登入 codex、更新 Hermes、重啟 gateway 等方式都無法解決。此 issue 已關閉。
